Thank you for your participation in the Ten Toe Express program. We are pleased to inform you over 4000 participants have registered since 2007. To help us evaluate the program, we are asking you to complete this follow-up survey regarding your experience with the spring session. While we strongly encourage you to complete the survey, your participation is voluntary and you do not have to answer any questions that make you feel uncomfortable. You will receive a $10 gift certificate to Schnucks by mail once you have completed the survey online. PLEASE COMPLETE YOUR SURVEY BY AUGUST 31, 2008. If you choose to fill out this survey, we will keep all information you provide us confidential. Your name and other information that identifies you will not be stored with your responses to the survey. We will use a project identification number to record your responses to the survey. Your privacy will be protected and your name will not be used in any materials. The fall session of the Ten Toe Express program will begin in September, 2008. We invite all of our existing participants to attend our fall kickoff, date to be determined.
If you are interested in re-registering for the fall session, please email the Ten Toe Express staff at tentoe@cmt-stl.org. Please include your name, address and email address in your email and include the subject line “Re-Register Ten Toe Express”. A member of the Ten Toe Express team will contact you with details about re-registering. Once you have re-registered for the program, you will also be able to participate in the fall walking groups. Keep in mind that you must complete this follow-up survey to re-register for the fall session. Fall walk groups begin the week of September 8th.
